This is the form we take on our initial client meetings. We gather all of their contact information on this form as well as: job notes, meeting dates, bid due dates, utility marking number if necessary, a job consideration checklist, and it even has a section on it to make it a contract if it is a simple project. 

Back when we were starting out we would have duplicates made of this form so if we did turn it into a contract we would leave one with the client and one for our records/scheduling!
